What Is Form FL-324(P)?
FL-324(P) is the Judicial Council’s mandatory professional-provider declaration. The current form is titled Declaration of Supervised Visitation and Exchange Services Provider (Professional).
What the Provider Declares
By signing the form, the provider declares that the mandatory professional-provider requirements have been satisfied. This is more than a training certificate: it addresses the provider’s overall eligibility and compliance.
Why the Academy Certificate Is Different
A Bridging Families Academy Certificate of Completion documents completion of the Academy’s training program. The Academy does not use the certificate to declare that the participant has completed Live Scan, obtained TrustLine registration, satisfied every background restriction, maintained insurance, or completed a court’s local administrative process.
Updated Forms With Court Reports
California law requires a professional provider to sign a separate, updated professional-provider declaration each time the provider submits a report to the court. Providers should use the current form version and follow the court’s procedure for submitting or maintaining the form and report.
Local Procedures Matter
Courts may establish procedures for processing and maintaining declarations and provider reports. Some referral lists or local programs may also require annual applications, supporting documentation, or continuing education. Check the current rules and instructions for the county involved.
